what happens to acids and bases in water

When we dissolve an acid in water it dissociates and in to ions.
HCl + H2O ------------> H+ + Cl-
and releases hydrogen ions.
when a base is dissolved in water it also dissociates and releases hydroxide ions.
KOH + H2O -----------> K+ + OH-
