In disease-free state, there is no appearance of the symptoms or effects of any disease. It refers specifically to absence of any infection or any abnormal condition that impairs bodily functions.

Health, on the other hand, is a state of well being wherein a person can function well physically, mentally and socially.

You have to be disease-free in order to be healthy. On the other hand, you can be in poor health but still be disease-free.

When we think about health, we think not only about an individual but also about societies and communities. But when we think about disease or disease-free, we talk at an individual level.

Being healthy is a relative term, varying for different people according to situations. Disease-free always refers to the state of absence  of a disease.
