Waveform perturbation refers to use of an external source to change the energy or displacement of a given wave i.e. to perturb a given wave. For example, when you throw a stone in water, you form waves and if you throw another one, you will distrub the waveform of the first one and form a new one. This is called as waveform perturbation.
