How is eye lens made according to the required defect of vision? What is its constituents?

The lens is made according to deffect , as for farsightedness we have to use a bioconvex lens , the focal length , we want a bio convex lens by which if an object is placed on the 25 cm , it appears that object is at near point of deffected eye. Means for that lens u=25 cm and v=near point of deffected eyea and now we can calculate the requried focal length.The desired lens can be made by lens maker forumla.
