The Ion exchange is mothing but theexchange of ions between the two electrolytes or in the electrolyte solution. Mostly it is used to denote the processes of separation, decontamination and purification of aqueous and other ion-containing solutions. 

Wboth ater contains calcium and magnesium ions which form salts.When it starts boiliing the water evaporates even a little and these cations precipitate. 

Because of this hard water also forms the scale in the water pipes and also in the boilers, both domestic and the industrial.
